Description:

Acne Vulgaris is one of the most common skin conditions affecting nearly 85% of adolescents.

Tiny non inflammatory acne lesions may not be more than a minor annoyance, but many suffer from the harsher inflammatory nodular acne with accompanying redness, soreness and possible scarring.

Acne starts as a microscopic lesion called the microcomedo. This lesion is characterised by follicular plugging of the duct of a pilosebaceous unit which consists of a hair follicle and a sebaceous gland. Sebum is produced in the pilosebaceous unit and when some of it is trapped beneath the keratin plug along with more keritanaceous material, this results in an enlargement of the follicle.

At this stage, despite this follicular plugging, apart from the natural hyperactivity of the sebaceous glands, there are no obvious signs of Acne or inflammation and neither is there any proliferation of the Propionibacterium Acnes Bacteria.

As these early lesions increase, they progress into a non inflamed closed or open comedo - either a whitehead or blackhead.

In time, these comedo may be attacked by bacteria, predominantly the Propionibacterium Acnes Bacteria, which in turn leads to larger inflammatory lesions such as papules, pustules and nodular cystic lesions characteristic of Acne.

The causes of Acne are still uncertain, but the strongest leads implicate androgen hormones, alterations to follicular linoleic acid levels, genetic predisposition, stress and diet.
